It simply was an accident and couldn't be avoided anywhere along that event chain.

It was negligence, and it was eminently avoidable. I recommend the following non-political publication:

Actually, taking the entire course would be preferable, but in lieu of that, an afternoon spent with the manual will go a long way toward avoiding such tragedies.
