this is... is a sign that something Not good is going on inside those Ugly innards. 
The damage to tissues causes this. But what all keep going nuts about is the thick smear of concealing makeup that does anything but. While I'm not makeup expert, I do know that if you have bruising, you need to first deflect the blue purple coloring before using any foundation/concealer. Typically GREEN neutralizes the dark purple (red tints) which then allows for the application of a foundation/concealer that more closely matches his (repulsive) skin and then light matching/translucent powder to set.
But, no one bothers to tell him this. (Nor would I, frankly) 
But, that is why it always looks so horrible. While the public thinks it has gotten worse, I have seen enough of this to argue it is his horrible "cover-up" that makes it look like his hand is rotting off rather than a dramatic change in the bruised hand itself. That said, the extravasation and bruising appears to have occurred multiple times (perhaps even ongoing--at least alternating hands). There are other veins available, but I suppose he'd go nuts even suggesting those in some locations--including the top of feet.
I really needed that first thing on Tuesday morning 🤮
