Trump has completely cathected on this threat: he thinks it is the one that will move the needle. "I can and will on a single word destroy the Kharg Island oil port!"

He has dementia - I'm sure of it. My experience with caring for elderly people with people w/dementia tells me he is losing it. My Dad - my dad could fool people but he would get so angry, so irrational, so unpredictable. It was a slow degeneration but he became a danger to himself and others.
